Une \u00e9quipe de chirurgiens-dentistes et de chercheurs en biom\u00e9canique Cor\u00e9ens ont \u00e9tudi\u00e9 les effets ind\u00e9sirables dentaires et squelettiques li\u00e9s au port d\u2019une orth\u00e8se d\u2019avanc\u00e9e mandibulaire.<\/p>

L\u2019objectif de l\u2019\u00e9tude a \u00e9t\u00e9 d\u2019\u00e9tudier les effets biom\u00e9caniques de l\u2019orth\u00e8se \u00e0 diff\u00e9rents degr\u00e9s de protrusion mandibulaire.<\/p>

Les forces musculaires cr\u00e9\u00e9es par l\u2019avanc\u00e9e mandibulaire ont \u00e9t\u00e9 mesur\u00e9es par des capteurs de pression fix\u00e9s sur l\u2019orth\u00e8se. Les mesures ont \u00e9t\u00e9 effectu\u00e9es pour des propulsions de 10 \u00e0 70% de la protrusion maximale du patient. Un mod\u00e8le biom\u00e9canique 3D a \u00e9t\u00e9 mod\u00e9lis\u00e9 \u00e0 partir de coupes de scanner en utilisant la m\u00e9thode des \u00e9l\u00e9ments finis.<\/p>

Les principales mesures recueillies correspondaient aux forces appliqu\u00e9es au ligament dentaire et \u00e0 l\u2019os spongieux. Cette analyse a permis d\u2019\u00e9valuer les risques de d\u00e9placement dentaire, r\u00e9sorption radiculaire et r\u00e9sorption osseuse.<\/p>

La mesure des forces \u00e9tait le plus faible \u00e0 40% de la propulsion mandibulaire active et le plus \u00e9lev\u00e9 \u00e0 70%. Le risque le plus important de d\u00e9placement dentaire a \u00e9t\u00e9 retrouv\u00e9 pour les molaires mandibulaires. La r\u00e9sorption radiculaire et osseuse est plus importante pour les secondes molaires.<\/p>

Les risques de d\u00e9placement occlusaux sont fr\u00e9quents, et seraient pr\u00e9sents \u00e0 long terme dans plus de 26% selon Clark. Cette \u00e9tude confirme les donn\u00e9es cliniques de l\u2019\u00e9tude d\u2019Anitua qui retrouve une corr\u00e9lation entre l\u2019importance de l\u2019avanc\u00e9e et la survenue d\u2019un d\u00e9calage occlusal.<\/p>

Ces donn\u00e9es incitent \u00e0 s\u00e9lectionner les patients dont la correction du SAOS est obtenue avec une avanc\u00e9e mod\u00e9r\u00e9e afin de limiter le risque d\u2019effets secondaires occlusaux.<\/p>
